All information provided is true, complete, and accurate to the best of my knowledge. I understand that providing false or misleading information may affect the accuracy or validity of the testing process. - Consent to Testing
All parties involved must provide informed consent before any collection or testing is performed. - Legal Compliance
You must acknowledge to being either a testing party or a legal guardian of a minor being tested. Only the parties listed for testing may enter the testing site unless otherwise authorized by PurePath Diagnostics. - Privacy and Confidentiality
PurePath Diagnostics will maintain the confidentiality of all personal and genetic information in accordance with applicable privacy laws. Information will only be shared with authorized individuals and the testing laboratory. - No Guarantees on Results
PurePath Diagnostics works with accredited laboratories, no guarantees are made regarding the outcome or conclusiveness of the results. Retesting may be required in some cases at an additional cost. - Non-Refundable Policy
Once testing has been performed or a kit has been shipped, the service becomes non-refundable. - Use of Results
Results may be used for legal, personal, or informational purposes as will be indicated on the intake form. It is the responsibility of the client to specify the intended use during submission. - Same-Day Service and After Hour Service
